Please fill out this form with all of your information so we can share your great work with the community!

After filling out the form you will be directed to a checkout page where you can make the $50 payment of $50 to secure your company's spot as a wellness partner.

In exchange, Ivy will be providing:

  • A dedicated pre-event post announcing our collaboration

  • Story tags leading up to the event

  • Tags on post event social media posts

  • Verbal acknowledgment during the event and opportunity for you to speak as well

  • Opportunity to bring your business cards and flyers for people to take

  • Any photos/videos from the event

After submission you will be led to a check out page where you can complete the payment to become a wellness partner.
